For Memorials and Honorariums

Please provide us with:

  • Name of person donation is in honor or memory of
  • Name and address of person who we should notify of your gift
  • Name and address of donor and who should be listed on the card

The CAP Center will promptly send a personalized card to the person or family indicated, notifying them of your thoughtful donation.

For General Donations

Please provide us with:

  • Your name, address, and email.

Every gift is tax-deductible to the fullest extent of the law and the donor will receive an acknowledgment for his or her donation.
